Tournament Fee Reimbursement Form 2025

Cayuga Minor Hockey will front the cost for 1 tournament for each Local League team, up to $1000. This will allow coaches to register for tournaments and not be out of pocket until they are able to collect funds or do team fundraising. Complete this form to automatically notify the Treasurer and Local League Convenor. Fields with * are mandatory.

Our Treasurer will notify you when the cheque is ready to be picked up at the hockey office.

This money is to be paid back to Cayuga Minor Hockey at least 2 weeks before your team attends the tournament or by December 1st, whichever comes first.

No requests will be processed after November 1st.
