Team Fundraiser Approval Form 2024-25
Use this form to seek approval for your team's fundraising/sponsorship plans from the Cayuga Minor Hockey Board. This request will be reviewed at the next board meeting - 2nd Wednesday of every month. If you're planning a fundraising event, please submit this form prior to the event, allowing sufficient lead time for review at the monthly Board meetings.
A Team Fundraising Report must be submitted to the Board within 2 weeks after every fundraising activity.
