Use this form to seek approval for your team's fundraising plans from the Cayuga Minor Hockey Board. This request will be reviewed at the next board meeting - 2nd Wednesday of every month. If you're planning a fundraising event, please submit this form prior to the event, allowing sufficient lead time for review at the monthly Board meetings.
