Cayuga Minor Hockey will front the cost for 1 tournament for each Local League team. This will allow coaches to register for tournaments and not be out of pocket until they collect funds or do fundraising. Complete this form to automatically notify the Treasurer and Convenors. Our Treasurer will provide a cheque that can be picked up in the hockey office.. This money is to be paid back to Cayuga Minor Hockey before you attend the tournament or by December 1st, whichever comes first. Fields with red * are mandatory
