Awards Nomination Form
Cayuga Minor Hockey Awards Night is Friday, April 24th in the Cayuga Secondary School cafeteria.
Many of the awards are selected by the teams' coaching staff, but a few are open to be nominated by the membership of Cayuga Minor Hockey.

Any Cayuga Minor Hockey Member can use the Awards Nomination Form (also available on the Forms tab) to nominate worthy candidates for the awards listed within.
Deadline is April 1st, 2026.
